A hole in the tooth is caries
A 'hole in the tooth' is the visible result of caries. Bacteria convert sugar from food into acid, which gradually demineralises the enamel. If the process isn't stopped, a real cavity forms — a hole that reaches deeper and deeper over time, all the way to the tooth nerve.
How do you recognise caries?
The tricky part: caries doesn't hurt for a long time. Watch for early signs:
- White, chalky or brownish discolouration on the tooth
- Sensitivity to sweet, cold or hot things
- A rough spot or a visible or palpable hole
- Pain — usually only when the caries is already deep
Because pain comes late, regular check-ups are so important — with an X-ray we also find caries where you can't see it, and before it reaches the nerve.
How is a hole in the tooth treated?
The earlier, the gentler and cheaper. The treatment depends on how deep the caries already reaches:
- Early caries (enamel only)Often still stoppable without drilling — with intensive fluoridation and good oral hygiene the enamel can partly remineralise.
- Small to medium holeThe carious area is removed and filled with tooth-coloured material — a <a href="/en/behandlungen/zahnkronen">filling</a> from CHF 300.
- Large defectIf too little healthy substance remains, a partial crown or <a href="/en/behandlungen/zahnkronen">crown</a> (from CHF 1’900) protects the tooth.
- Down to the nerveIf the caries reaches the pulp, a <a href="/en/blog/wurzelbehandlung-ablauf-schmerzen-kosten">root canal treatment</a> becomes necessary — usually followed by a crown.

How to prevent caries
- Brush twice a day with fluoride toothpaste — and clean the interdental spaces daily.
- Reduce sugar and acidic drinks and don't snack constantly in between.
- Regular professional cleaning — it removes plaque and tartar where the brush can't reach.
- A check-up once or twice a year — so caries is detected early.
